Youth Sign Up - Bowling & Pizza

Get ready for an epic Youth Bowling & Pizza Night on Friday, June 21st at Palasad South, 141 Pine Valley Blvd, London! The fun starts at 7 PM and goes until 9 PM. You will need to bring $35 for bowling, but don't worry about the pizza – it's on us! Plus, after pizza, get ready for a special bonus: we'll cover one game of laser tag too!

Parents, please fill out this form for your household including the name and health card number of each youth. If bringing friends, please have their parents fill this form out.

I/we, named above, undertake and agree to indemnify and hold harmless Program Personnel, Aylmer Full Gospel Church, and its leaders from and against any loss, damage or injury suffered by the participant as a result of being part of the activities of Aylmer Full Gospel Church, understanding that your youth’s safety is our priority.

In the event of an emergency, and the contact persons listed above cannot be reached, I authorize the AFGC as an agent for myself to consent to treatment and hospital care advised and supervised by a physician.

I give permission to AFGC to utilize any photos taken during this activity for photo collages, updates shared with parents, and future promotions only.
